INFORMATION AND CONSENT TEXT
In order to carry out the services to be provided to you by our dental clinic, we may need to learn your personal information and health data and record and store them within the limits required by the service to be provided.
Your health data, which we must keep track of in order to provide you with health services, are classified as special personal data under the law. In this context, it is mandatory to obtain your consent under paragraph 2 of article 6 of the Law on the Protection of Personal Data No. 6698, which states, “It is prohibited to process sensitive personal data without the explicit consent of the person concerned.” Personal health data can only be recorded with the person’s explicit written consent, except under special circumstances specified in the law.
INFORMATION TEXT
This consent includes your personal data that you provide to us verbally, in writing, visual or electronically in our clinic, and your personal data (analysis result, prescription, camera recording, video, photograph, etc.) that you transmit to us via the internet and mobile applications or electronically or obtained in our practice.
Your name, surname, TR identity number (if you are not a Turkish citizen, your passport number or temporary TC identity number), place and date of birth, marital status, gender, also your contact data such as your address, telephone number, e-mail address, your bank account number, financial data such as IBAN, your medical history in your clinical file, information showing your disease history, your examination data, data regarding the procedures applied to you, your prescription information, photos, all kinds of images and sounds, your data on health and sexual life obtained during the execution of medical diagnosis, treatment and care services such as your camera recording, laboratory and imaging results, test results, data on private health insurance, data from the Social Security Institution, etc. is considered personal data. This personal and health data is necessary to provide the dental services we will provide to you and are obtained for this purpose only.
Your personal data will be recorded only to the extent required by the health service to be provided to you within the framework of the Law on Protection of Personal Data No. 6698 and the relevant legislation. They will be stored in our system/archive for a period not exceeding the period required to achieve recording purposes. In this context, your processed data will be protected as a professional secret; we will ensure confidentiality. It will not be shared with third parties/institutions/or organisations.
However, in cases where the privacy of personal medical records must be restricted for the protection of public health, as in the example of the obligation to notify the competent authorities of infectious diseases regulated in Article 58 of the General Sanitary Law No. 1593 or in cases of a legal obligation such as the obligation to report the crime, only limited to the purpose and we strongly remind you that it may be necessary to notify the competent authorities in a measured manner or that it may be shared with another physician for the purpose of consultation (exchange of opinion) regarding your health condition.
Requests from public institutions, judicial authorities and other official authorities for the transmission of your data to them, the purpose of the request, whether the requested data overlaps with the purpose to be achieved, whether it can be put forward concretely, the only way to achieve the stated purpose is the necessity of transmitting your data without anonymisation, data transmission requests that do not meet all of these elements will not be fulfilled.
Regarding your data recorded by us, in accordance with the Convention on the Protection of Individuals against Automatic Processing of Personal Data (Convention of the Council of Europe No. 108), Article 8 of the European Convention on Human Rights, Article 20 of the Turkish Constitution, Law No. 6698 on the Protection of Personal Data, you have the right to:
- To learn whether your personal data is processed and the scope of your processed data,
- To learn if your personal data has been processed, obtain information about it, access these data and take samples from them,
- To learn the purpose of processing your personal data and whether they are used in accordance with its purpose,

- To request correction of your personal data if it is incomplete or incorrectly processed,
You have the right to request that some of your data be hidden, deleted or destroyed.

I have been given verbal information about the subject, the purposes of processing my personal data, collection methods, and legal reasons, which are detailed in the Personal Data Clarification and Consent Text. I have been informed of my rights, the mandatory conditions under which my data can be transferred, data security, and my rights to apply, and that all of my personal data, including my health data, will be recorded, stored, and shared in mandatory cases by Private Müge Ateş Bodrum Oral and Dental Health Polyclinic and its employees in accordance with the above principles, and that I have been informed of the following conditions. Müge Ateş can contact me by mobile means, on the internet or by mail to my address, etc. I AGREE WITH MY EXPRESS CONSENT.
In accordance with the Patient Rights Regulation, one copy of the form will be given to the patient. If the form is not given to you, please notify the person receiving the consent.
